Managers are required to complete a formal employee evaluation form at least once a year for each of their staff.  This particular document becomes a big part of each staff member's permanent personnel file.  It is used to determine the employee's eligibility for promotion and raises.  Learning how to write an employee sample report is mandatory for every supervisor and manager.  Always ensure that you review all the employee evaluation forms you write to ensure they are well written.  For those who cannot write out even a simple evaluate form, here you

EXCEPTIONAL
The worker's overall performance that consistently surpasses all
performance criteria established for the position. This evaluation recognizes an employee's continued excellence and achievements which are substantially above typical expectations.
EXCELLENT
The staff's overall performance in all areas regularly exceeds the performance measures set for the position. This evaluation notices an employee's consistent effectiveness and attainments which are above normal expectations.
SATISFACTORY
The employee's overall production consistently meets the performance standards placed on the position and regularly produces expected results. An employee at this performance level meets common expectations and completes tasks in a timely and acceptable manner.
UNSATISFACTORY
The employee's overall achievement unpredictably meets the performance standards settled for the position and registers that important tasks are not completed in the time or way expected. Performance is below the minimum acceptable level for the position.  Improvement of performance deficiencies is necessary for continued hiring.

Note: 
The employee should be called aside for a one on one meeting to discuss their performance with their supervisors without fail. 
The supervisor should ensure every report is signed by the employee before it is filed for accountability reasons.
