What to include in the employee evaluation form doc

Employee details

Here you should include the name of the employee, position, their department, and the exact date of the evaluation. It is important to understand that every employee should be assessed using a copy of the form.

Instructions/key

The next component of the evaluation form is the instructions. They direct the evaluator on the rating scales, which they are required to use in grading the employees. Furthermore, it points where they should check or use yes/no.

Performance criteria

In this section, you should include each criterion, which you will be using for the employee evaluation. Common categories include job knowledge, punctuality and attendance, customer service, working relations, quality of work, dependability, adaptability, initiative and innovation, leadership, and communication skills. To conduct the 360-degree evaluation, having as many categories as possible can be beneficial.

Evaluator/ Employee’s comments

The evaluation form should leave a space for the employee’s comments and signature. The evaluator also needs to present their views about the evaluation and sign the form. This section summarizes all the observations that the evaluator made during their assessment. On the other hand, the employee’s comments section gives the employee a platform for clarifying issues.
